Form Filling Instructions

For researchers staying in Prague: When collecting all these documents, please come to the office of the Czech Services Center where you will leave them and after signing a power of attorney, we will submit the documents for you at the Alien Police Department. When your residence permit is ready, we will inform you about the exact date and time, so you can pick up your visa without queueing.

Change of temporary address in the Czech Republic (obligation to notify the Alien Police Department (APD) within 3 days from the date of the address change)

Documents needed:

1. Form for the address change (available at the APD or at the Czech Services Center - CSC)

2. Original (APD will keep one) of the new contract or confirmation of accommodation (the form available at the CSC)

3. Travel document (passport)

Registration at the Alien Police Department – for non-EU nationals staying in the Czech Republic for the purpose of scientific research more than 90 days (obligation to register within three days from the date of arrival to the CR)

Documents needed:

1. Application form for a foreigners ID number (form available at the APD or at the CSC)

2. Registration form (available at the APD or CSC)

3. Travel health insurance

4. Travel document (passport)
